The fee for any of conversion type is $50.00 and may be paid online through the payment page links listed below. 

CONVERTING LICENSE TYPE

Temporary to Standard

Audiologists & Speech-Language Pathologists

Visit our Conversion Payment page to pay the fee and Board staff will make the change. You will be notified via email when the conversion is complete. 

Dispensing Audiologists & Hearing Aid Specialists

  1. Wait for notification of passing scores on both the Written and Dispensing Practical Examinations from the Board.
  2. Upon receipt, visit our Conversion Payment page to pay the fee and Board staff will make the change. You will be notified via email when the conversion is complete. 

Provisional to Standard

Speech-Language Pathologists

  1. Email a copy of your ASHA Certificate of Clinical Competence to board@nvspeechhearing.org.
  2. Visit our Conversion Payment page to pay the fee and Board staff will make the change. You will be notified via email when the conversion is complete

Hearing Aid Specialists

  1. Email a copy of your NBC-HIS certification to board@nvspeechhearing.org.
  2. Visit our Conversion Payment page to pay the fee and Board staff will make the change. You will be notified via email when the conversion is complete

HAS Apprentice to HAS

  1. Download the Apprentice Training Completion & Competency Evaluation form. This must be completed and signed by both Sponsor and Apprentice. Email to the Board at board@nvspeechhearing.org.
  2. Wait for award of NBC-HIS certification and notification of passing scores on both the Written and Dispensing Practical Examinations from the Board.
  3. Visit our Conversion Payment page to pay the fee and Board staff will make the change. You will be notified via email when the conversion is complete. 

CONVERTING LICENSE STATUS

Active to Inactive

  • An active license may be converted to inactive status ONLY at the time of renewal. 
  • The change to inactive status requires an attestation from the licensee that they will no longer practice or represent to others that they are authorized to practice after the license has been converted to inactive status.
  • Once converted, the license must still be renewed annually (for a lesser fee), and per NAC 637B.0368 the licensee must still complete the same annual Continuing Education requirement as they would for an active license.
  • A licensee who does not plan to practice for the foreseeable future may also choose to simply let their license expire, which would remove the need to renew and complete CEs, and would have up to three (3) years to apply for a license reinstatement. 

Inactive to Active

  • An inactive license may be converted to active status at any time, including at the time of license renewal.
