All sections of the form are mandatory and must be completed to process the request. The mailing list will be sent as an Excel Spreadsheet via E-Mail. Please allow 2-3 business days for processing. If you have any questions you may contact the board office at (775) 787-3421. Please note that the mailing list will ONLY include names and mailing addresses. Telephone numbers and email addresses are NOT included.
